Forms for Adventure Day Camp

This form is for our Adventure Day Camp visits to Jones Gulch.

The following form ONLY needs to be filled out if you are attending Adventure Day Camp during week 1 (6/5-6/9) and/or week 3 (6/19-6/23). Once filled out, upload this to your CampBrain account in your “Submitted Application” section. This waiver applies to all Adventure Campers, whether you chose the Optional Campout or not.

*This waiver may be uploaded to your Cambrain account or brought the first day of camp. Please do not send this directly to a Run for Fun email.
*If you are only signed up for Run for Fun Day Camp, you do not need to fill out this form.
